Photo identification cards “card” are
issued to all employees upon date of hire.
Cards are valid for -year periods and are reissued at the
end of each period until employment terminates.
Replacement of lost cards will be
issued upon payment of a service fee.
Unauthorized use of a card could result
in disciplinary action, up to and including termination.
Terminating employees must turn in
their card to the supervisor or Human Resources.
All photo identification cards are
issued by Human Resources during regular business hours.
