Short Term Disability

Policy Rationale

To provide income replacement for employees who are unable to work due to a non-work-related illness or injury.

Eligibility for Benefits

In order to access benefits, employees must:

·         Be absent for more than .

·         Be disabled or injured while actively employed or within enter number of days days after termination.

·         Provide medical evidence of their inability to work due to illness or injury.

Description of Benefits

During the first enter number of days  days of absence employees must use accrued sick leave.  Employees who have insufficient sick time to cover this waiting period are put on an illness leave without pay for the balance of the enter number of days days.

After the waiting period, employees whose medical evidence is approved will receive disability benefits at  of their regular pre-disability earnings. The disability benefit will be topped up with accrued sick leave until the return to work or sick leave is exhausted.

A return to work slip must be provided to HR before an employee is allowed to return to work.

Employees who return to work part-time will have their disability benefit reduced by their actual earnings.  If an employee earns more than the disability benefit, the reduction will also come from his/her accrued sick time top up.

Employees who exhaust accumulated sick leave and cannot return to work will continue to receive disability benefits.  Employees have the option of using accumulate vacation to top up the benefit.

Benefits will continue for the period of disability up to a maximum of enter number of weeks weeks from date of disability.

Benefits for temporary employees are as follows:

Participation

Permanent employees are eligible upon date of hire.  Temporary employees must be employed for at least  by Employer ABC in order to become eligible.

General Provisions

Medical Evidence or Physician's Certificate: Payment of benefits is contingent upon certification of illness or disability by a physician. Absences of more than enter number of days  days must be substantiated by medical evidence.

Sick Reports: Departments must submit a Sick Report to Human Resources for employees who are absent from work for more than enter number of days  days due to a non-work-related illness or injury.

Holiday Pay: Employees on short-term disability leave during a holiday will receive holiday pay in lieu of disability leave, sick leave or vacation leave.

Pending Determination of Long-Term Disability Status: At the end of the maximum enter number of weeks weeks of short-term disability, employees are required to apply for long-term disability.  Upon approval, long-term disability benefits are retroactive to the date short-term disability benefits ceased.  In order to minimize the waiting period between short-term and long-term disability payments, employees are encouraged to submit medical evidence four weeks prior to the cessation of short-term benefits.

Vacancy Limitations

While an employee is on short-term disability, his/her position cannot be filled on a permanent basis.
